WAIVER OF LIABILITY

The client will hereby understand and acknowledge that the training, programs and events held by the Lion’s Den Fitness and Training may expose clients to many inherent risks, including accidents, injury, illness, or even death. The client will assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by the client. The client will hereby acknowledge the responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. The client will acknowledge that they are physically fit and mentally capable of performing the physical activity they choose to participate in. After having read the waiver and knowing these facts, and in consideration of acceptance of participation and the Lion’s Den Fitness and Training furnishing services to the client, will agree, for all participants entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE the Lion’s Den Fitness and Nutrition, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in the Lion’s Den Fitness and Nutrition programs, trainings, and/or events.


GROUP FITNESS CLASS

Congratulations on your decision to participate in an exercise program! With the help of your personal trainer, you greatly improve your ability to accomplish your training goals faster, safer, and with the maximum benefit. The details of these training sessions can be used for a lifetime!


In order to maximize progress, it will be necessary to for you to follow program guidelines during supervised training days. Remember, exercise is only part of the health equation. Nutrition, stress management, and sleep are also some of the factors that can play a factor into your health.


During your exercise program, every effort will be made for you to assure your safety. However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. In volunteering for this program, you agree to assume responsibility for these risks and waive any possibility for personal damage. You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude and exercise program.


By signing below, you accept full responsibility for your own health and well-being and you acknowledge and understand that no responsibility is assumed by the leaders of this program.


It is recommended that all participants do their best to attend the classes/training sessions on a weekly basis.

Terms and Conditions

  • Group classes are set up in a circuit style to an interval of set time. The class is designed for the participant to self-modify exercises and/or determine the intensity of the exercise.
  • Clients who arrive late will be responsible for a proper warm up before joining the other participants.
  • All classes are paid in full at the beginning of the month. Late payments can be subject to a $10 late fee.
  • No refunds will be issued for any reason, including but not limited to non-attendance, acute illness, or missed classes.

Total Investment: $120 per month

Method of Payment: check, cash, Zelle, CashApp.

NUTRITION POLICY

The client will hereby consent to and authorize clinician of the Lion’s Den Fitness and Nutrition to provide professional healthcare services on behalf of the client. The client further understands that the healing therapies I am endeavoring into are based upon holistic protocols, and may involve nutrient, supplement, and/or herbal recommendations.  These protocols are intended to improve energy, alleviate pain, and restore body and spirit for long-term healing resolutions.   

          
The client recognizes that sometimes a healing response may occur at the initiation of holistic protocols.  A healing response tells that the protocols being utilized have produced changes that the body is now integrating as health improves toward the client and provider’s mutual goals.                
              
As this is a voluntary endeavor, the client has the right to discontinue treatments at their discretion and will discontinue any treatments that they feel uncertain of until such time as they can speak with their clinician and confirm the safety of continuation.

As a new Client, I agree to:

  • Fully disclose all physical and/or psychological health conditions that may be necessary for the clinician to know in order to assure their safety, honor their emotional history, and allow their healing team to provide them with the best possible healing experience.
  • Inform the clinician immediately, via phone or email, of any physical or emotional discomfort or pain too intense to manage or that lasts for more than 2 days (48 hours) following a treatment.

The client will hereby authorize the clinicians and support staff to perform the following specific procedures as necessary to facilitate the assessment and design recommended treatments:

  • Holistic use of nutrition: Therapeutic nutrition and nutritional supplementation.
  • Botanical medicine: Botanical substances may be prescribed as teas, alcohol or glycerite based tinctures, capsules, tablets, creams, plasters, or suppositories.
  • Lifestyle counseling and hygiene: Diet therapy, promotion of wellness including recommendations for exercise, sleep, stress reduction, and balancing of work and social activities.

The client will recognize the potential risks and benefits of these procedures as described below:  
           
Potential risks: allergic reactions to prescribed herbs and supplements, side effects of natural medications, aggravation of pre-existing symptoms, discomfort, pain, infection, burns, nausea, light headedness, inconvenience of lifestyle changes. Please notify the clinician if you experience any symptoms which may be secondary to the above procedures or if ever in doubt.        
           
Potential benefits: restoration of energy, health and the body’s maximal functional capacity without the use of pharmaceuticals or surgery, relief of pain and symptoms of disease, assistance in injury and disease recovery, and prevention of disease or its progression. Notice to pregnant women: All female clients must alert the clinician if they know or suspect that they are pregnant as some of the therapies used could present a risk to the pregnancy, and should talk with their MD before implementing any protocol recommendations.                 
       
With this knowledge, the client voluntarily consents to the above procedures, realizing that no guarantees have been given to the by this office or the clinician, therapeutic staff, or representatives regarding cure or improvement of my condition. The client understands that they are free to withdraw their consent and to discontinue participation in these voluntary procedures at any time.  The client is responsible for all actions they take, and do not hold Christina Valderas or Lion’s Den Fitness and Nutrition responsible for their actions.  The client understands any choices they make should be discussed with their doctor first.


Cancellation for Personal Training Clients

Purpose: Lion’s Den Fitness and Nutrition (LDF&N) understands that personal emergencies may interfere with clients being able to keep an appointment time for a scheduled personal training session. In consideration of these unforeseen circumstances, this cancellation policy has been developed to create fairness to both client and trainer.


Policy: LDF&N personal training staff requires a 12-hour notice to cancel any scheduled training session. Cancellation must by acknowledged by the trainer. Clients will receive one free short-notice cancellation (short notice is less than 12 hours prior) without being charged a cancellation fee in a 6-month period.


If you do not appear for a scheduled appointment or fail to give 12 hours’ notice, you will be charged for the full session. You must receive confirmation of cancellation from the trainer. Clients who do not initiate contact for a cancellation are not eligible for the free cancellation. Procedure: Phone, text, or email confirmations will be accepted and should be responded to within two hours of receipt by the personal trainer.

Please arrive on time. Training sessions will end at the pre-arranged time-slot regardless of starting time. Trainings sessions may not be able to be extended if the client arrives late as the trainer may have other sessions immediately following your session.
